Paper forms are still available if you need one. You can download the form here. Once completed bring the form to the practice between 10am to 12pm and 2pm to 4pm every weekday, preferably with a medical card, proof of ID and address.
Many of our patients have expressed the view that we are registering too many patients. As NHS GPs we are bound by our contract to register any patient who live in our catchment area and wish to register with us.
We cannot change our catchment area unless we have permission from the government.

Registration Forms
When filling in the registration form it is important that the form is fully completed to comply with the Health Authority requirements:
- Previous surname (if any)
- Town and country of birth
- Home address with postcode
- Telephone number
- Previous address
- Name and address of previous GP
- If appropriate, date of arrival in the UK
Once the forms are submitted, reviewed and approved by our team, you will be registered within 7 working days.
